2. At the top of your application be sure to include your name, address, phone number, email address, and if this is for a new scholarship or a renewal.
3. Please be sure to label each section by using the titles below.
4. Scholarship application is due by April 1, 2025.
Service:
Please number and list any services you have provided in the past two years. Include what the service is/was, who was served, and when the service occurred.
Activities and Leadership:
Please list any activities in which you are currently involved, such as in school, community, job, church, sports, organizations, clubs, etc. If you hold any offices or leadership roles, please include those.
Awards/Recognitions Received:
Please name three achievements, goals, and/or awards of which you are most proud.
Personal Plans:
For this section, please tell us what is or will be your area of study for your degree and what is your future career goal.
Additional Comments:
Briefly tell us if there are any special circumstances or considerations you would like us to know?
Academics:
Please include your official transcripts along with your application.
References:
Please include two letters of recommendation from persons not related to you, such as bosses, pastor, school counselors, coaches, and leaders of the community, clubs, and organizations.
